Temagami Family Health Team is recruiting a full-time Physiotherapist
We are currently seeking a motivated and organized Physiotherapist to join our team on a permanent full-time basis. This individual will provide service to patients of the Temagami Family Health Team, with clinic sites in Temagami, Cobalt and Latchford. The main clinic site for this position will be the Latchford Medical Centre.
Working as a physiotherapist on a multidisciplinary team, you will:
• Collect and review pertinent information from the EMR (electronic medical record) and patient/caregiver interviews
• Be responsible for the delivery of patient-centred physiotherapy services;
• Provide assessment, goal-oriented treatment interventions, patient/family education and support to address medical and emotional needs
• Participate in program planning , continuous quality improvement and evaluation
• Develop and discuss appropriate individualized care plans, based on best practice, with patients/families/caregivers, in collaboration with the primary care providers and other members of the team
• Provide ongoing case management for patients
Qualifications:
• Registered with the College of Physiotherapists of Ontario;
• Graduate of a recognized university physiotherapy degree program;
• ADP authorization would be a strong asset;
• Experience with health promotion and prevention, chronic disease management and adult education considered an asset;
• Experience in pelvic health physiotherapy would be an asset;
• Proven organizational, observational, interpersonal and communication skills;
• Strong clinical thinking and problem-solving skills;
• Familiarity with Electronic Medical Records;
• Current Ontario Driver’s License;
• Evidence of liability insurance
• Oral proficiency in French at an advanced or superior level considered an asset
Salary is $82,340 plus a comprehensive benefits plan and enrolment in the organization’s HOOPP pension plan.
